If confirmed that you are HIV positive (from a doctor, health department or from us) a referral is made to our case manager. The case manager conducts a thorough assessment to see what services you may need and will work out a care plan to aid in your compliance. We provide referrals to other community services to assure you get all the help you need. W ith your consent, we also can meet with family members to help guide them through the ways they can help you. We offer a full Continuum of Care who can help with: Case Management, Medical Case Management, Medication Assistance, Insurance Assistance, Outpatient Medical Care, Housing Assistance, Food Pantry, Transportation Services, Nutritional Therapy, Mental Health Services, Dental Services.
