If confirmed that you are HIV positive (from a doctor, health department or from us) a referral is made to our case manager. The case manager conducts a thorough assessment to see what services you may need and will work out a care plan to aid in your compliance. We provide referrals to other community services to assure you get all the help you need. W ith your consent, we also can meet with family members to help guide them through the ways they can help you. We offer a full Continuum of Care who can help with: Case Management, Medical Case Management, Medication Assistance, Insurance Assistance, Outpatient Medical Care, Housing Assistance, Food Pantry, Transportation Services, Nutritional Therapy, Mental Health Services, Dental Services. Our food pantry is available Monday through Friday. We are proud of our commitment to provide support, education and testing. All Services are Strictly Confidential.
