Requirements Needed:
1.
Proof of income,
2. Birth certificate of the child,
3. Child health and immunization results,
4. Social security cards for children and parents and
5. Proof of address.
Must meet one of the following criteria:
federal income guidelines,
The child is three or four years old,
The child resides in the area,
The child is diagnosed with special needs,
The child was referred by another agency,
The child needs socialization, or
The child's siblings are currently enrolled.
