About Lake County Health Dept. Clermont Women, Infants & Children

A food & nutrition education program that provides nutrition & diet counseling, nutritious food, breastfeeding support & referrals for health care eligible women, infants & children. Age groups: Children under age 5, Infants, Women who are pregnant, breastfeeding or have recently been pregnant. Hours: Mon.-Fri. 8 a.m.-5 p.m. Fees: Free. Requirements: Each member to receive WIC must suppliy: Proof of ALL household income, OR Medicaid, Temp Cash Assistance/Food Stamps, proof physical address and ID; WIC medical referral form filled by your MD, and immunization record for each child. Languages: English, Spanish
